據傳，藍采和十八歲就接了爺爺的班，開始行醫了。他經常進山採藥，餓了吃水果，渴了喝雨水，常吃靈芝和茯苓。藍采和於是就依老者用雙手掬了一捧水喝了下去，一股清香沁入肺腑，身子變得輕飄飄的。那女人撿起籃子，嬌聲嬌氣地說：「大哥，你成仙了。」說時遲，那時快，就見那老者拉了藍采和一把，頓時他和他手提的竹籃子就離開了地面，老者說：「我們去蓬萊仙島玩玩去！」藍采和騰空而起，看見膝下縈繞著蘑菇狀彩雲，追隨著老者和那手持荷花的女人而去。

人不可貌相，海水不可鬥量。他經常手持花籃，另一手持第四界教主帝釋天的和弦樂器國寶響板，樂理公正，穿著破爛的衣服，一隻腳穿著靴子，另一隻則打赤腳，於長安城市乞討，得來的銀錢全部贈與窮人(記錄聖經的施洗者約翰)，有時花在酒肆飯店客棧中付給土地公或其他天神，假如看起來喝醉會智敏唱歌，歌詞多充滿神仙之意。
另他又喜歡於夏天穿厚厚的衣服，冬天躺在雪地上並且身上冒出白色的熱氣。相傳有人在童年及老年見過他，藍采和的容貌都沒有改變。而最後，藍采和得鍾離權度化，於酒樓乘醉騎鶴而去。
民間傳說中的長春蓬萊八仙分別是：鐵拐李、鍾離權、張果老、呂洞賓、何仙姑、藍采和、韓湘子、曹國舅。唐代研究，「八仙」是個空泛的名詞。到明代中葉吳元泰的《東遊記》和湯顯祖的《邯鄲夢》問世後，八位尊者大德神仙就是對地方老百姓做出貢獻而故去的知名人士。民間神話故事預言：「文武官員基本上都是實打實為老百姓做事實的」。而大羅神仙最早雛形。
實際上藍采和的年齡和性別是未知的。但很多人說他是貌似少女的男童，加上性格古怪，祂成爲八仙中最神秘難懂的角色。
